Institutional review boards are almost universally considered to be overworked and understaffed. They also require substantial commitments of time and resources from their members. Although some surveys report average IRB memberships of 15 people or more, federal regulations require only five. We present data on IRB meetings at eight of the top 25 academic medical centers in the United States funded by the National Institutes of Health. These data indicate substantial contributions from primary reviewers and chairs during protocol discussions but (...) little from other members, which implies that it may be possible for smaller IRBs to accomplish the same tasks with no reduction in the quality of review. The reasons for this are discussed and the possible objections examined. An attempt is made to see whether there are philosophical implications of this unexpected geometrical regularity. A new tetrahedral design in transparent plastic is presented.

BackgroundResponsive neurostimulation has been utilized as a treatment for intractable epilepsy. The RNS System delivers stimulation in response to detected abnormal activity, via leads covering the seizure foci, in response to detections of predefined epileptiform activity with the goal of decreasing seizure frequency and severity. While thalamic leads are often implanted in combination with cortical strip leads, implantation and stimulation with bilateral thalamic leads alone is less common, and the ability to detect electrographic seizures using RNS System thalamic leads is (...) uncertain.ObjectiveThe present study retrospectively evaluated fourteen patients with RNS System depth leads implanted in the thalamus, with or without concomitant implantation of cortical strip leads, to determine the ability to detect electrographic seizures in the thalamus. Detailed patient presentations and lead trajectories were reviewed alongside electroencephalographic analyses.ResultsAnterior nucleus thalamic leads, whether bilateral or unilateral and combined with a cortical strip lead, successfully detected and terminated epileptiform activity, as demonstrated by Cases 2 and 3. Similarly, bilateral centromedian thalamic leads or a combination of one centromedian thalamic alongside a cortical strip lead also demonstrated the ability to detect electrographic seizures as seen in Cases 6 and 9. Bilateral pulvinar leads likewise produced reliable seizure detection in Patient 14. Detections of electrographic seizures in thalamic nuclei did not appear to be affected by whether the patient was pediatric or adult at the time of RNS System implantation. Sole thalamic leads paralleled the combination of thalamic and cortical strip leads in terms of preventing the propagation of electrographic seizures.ConclusionThalamic nuclei present a promising target for detection and stimulation via the RNS System for seizures with multifocal or generalized onsets. These areas provide a modifiable, reversible therapeutic option for patients who are not candidates for surgical resection or ablation. (shrink)

Miroslav Volf initiates his theological inquiry by exammmg the contemporary situation of work, its transformation from a world of agricultural and industrial production into a system of information technologies. He attentively notes that this contemporary technology brings with it both advantages and disadvantages for workers. He also examines crises such as unemployment and discrimination which ac· company employment today. Despite both the ambiguity which accompanies the change in production and the threatening uncertainties caused by the current crises, he nevertheless emphasizes the duality of work: people consistently find their work to be both curse and delight. VoH wishes to emphasize this twin feature so that the theology he fashions wm he realistic and active. Volf pursues a theology of work which can account for varied developments in quite different economies and for the needs of succeeding generations. Thus Volf describes the activity of work in terms which are invariant with respect to time and place. By a theology which is adequate to this global concept of work, he means one " developed on the basis of a specifically Christian soteriology and eschatology. o. " (79). While Volf maintains an attention to the Christian traditions in recalling soteriology, he downplays a creation-based understanding of work in favor of one which is eschatological. Work is a cooperation with God in the transforming of the world on the way toward the new BOOK REVIEWS 709 creation of the end-time. Volf's laying-out the transformative character of work is insightful. It avoids the recurrent consequences of a theology of work based solely on creation: effort without limit and sullen acceptance of the toil. How does he accomplish this? First, he explores the lodes of dominant theoretical understandings of work by examining the.thought of A. Smith and K. Marx. Second, he presses for a pneumatological understanding of work. That is, he holds that rthe presence of the Spirit is key to human activity (133). Volf makes use of the theological tradition of the charisms in order to present more completely how human effort and divine activity can cooperate in work. The present study investigated whether scores on the Autobiographical Memory Test (AMT) in 55 patients with borderline personality disorder (BPD) were modified after long‐term psychotherapy and whether the pretreatment AMT scores would predict improvement in depression severity or BPD symptom severity at the end of treatment. In addition, it was analysed whether changes in ratings of mood, thought suppression, dissociation, and BPD symptom severity following treatment were associated with changes in AMT scores. Only patients with BPD and a comorbid diagnosis (...) of depression at time 1, generated significantly more specific memories and fewer categoric memories after 15 months of therapy. Moreover, these changes were unrelated to type of therapy and changes in depression severity, borderline symptom severity, dissociation, or thought suppression. The AMT scores at initial assessment did not predict depression severity at 15 months. The percentage of negative specific memories tended to predict BPD symptom severity. (shrink)

BackgroundOther Specified Feeding and Eating Disorders are characterized by less frequent symptoms or symptoms that do not meet full criteria for another eating disorder. Despite its high prevalence, limited research has examined differences in severity and treatment outcome among patients with OSFED compared to threshold EDs [Anorexia Nervosa, Bulimia Nervosa, and Binge Eating Disorder ]. The purpose of the current study was to examine differences in clinical presentation and treatment outcome between a heterogenous group of patients with OSFED or threshold (...) EDs.MethodPatients with threshold EDs or OSFED presenting for eating disorder treatment completed self-report questionnaires at intake and discharge to assess eating disorder symptoms, depression symptoms, impairment, and self-esteem.ResultsAt intake, OSFED patients showed lower eating concerns compared to patients with BN, but similar levels compared to AN and BED. The OSFED group showed higher restraint symptoms compared to BED, and similar restraint to AN and BN. Global symptoms as well as shape and weight concerns were similar between OSFED and threshold ED groups. There were no differences between diagnostic groups in self-esteem, depression scores, or symptom change from intake to discharge.DiscussionOur findings suggest that individuals with OSFED showed largely similar ED psychopathology and similar decreases in symptoms across treatment as individuals diagnosed with threshold EDs. Taken together, findings challenge the idea that OSFED is less severe and more resistant to treatment than threshold EDs. (shrink)

DBS Think Tank IX was held on August 25–27, 2021 in Orlando FL with US based participants largely in person and overseas participants joining by video conferencing technology. The DBS Think Tank was founded in 2012 and provides an open platform where clinicians, engineers and researchers can freely discuss current and emerging deep brain stimulation technologies as well as the logistical and ethical issues facing the field. The consensus among the DBS Think Tank IX speakers was that DBS expanded in (...) its scope and has been applied to multiple brain disorders in an effort to modulate neural circuitry. After collectively sharing our experiences, it was estimated that globally more than 230,000 DBS devices have been implanted for neurological and neuropsychiatric disorders. As such, this year’s meeting was focused on advances in the following areas: neuromodulation in Europe, Asia and Australia; cutting-edge technologies, neuroethics, interventional psychiatry, adaptive DBS, neuromodulation for pain, network neuromodulation for epilepsy and neuromodulation for traumatic brain injury. The essays in this book contextualize religion within a variety of cultural transformations. The methodological proposals of theologian and philosopher Bernard Lonergan, S.J. provide the inspiration and framework for these studies; and each makes its own distinct contribution beyond Lonergan's original work.

Current workflows in academic ecology rarely allow an engagement of ecologists with philosophers, or with contemporary philosophical work. We argue that this is a missed opportunity for enriching ecological reasoning and practice, because many questions in ecology overlap with philosophical questions and with current topics in contemporary philosophy of science. One obstacle to a closer connection and collaboration between the fields is the limited awareness of scientists, including ecologists, of current philosophical questions, developments and ideas. In this article, we aim (...) to overcome this obstacle and trigger more collaborations between ecologists and philosophers. First, we provide an overview of philosophical research relevant to ecologists. Second, we use examples to demonstrate that many ecological questions have a philosophical dimension and point to related philosophical work. We elaborate on one example – the debate around the appropriate level of complexity of ecological models – to show in more detail how philosophy can enrich ecology. Finally, we provide suggestions for how to initiate collaborative projects involving both ecologists and philosophers. (shrink)

To celebrate the four-hundredth anniversary of Galileo's birth, the University of Rochester held a series of lectures on the thought and influence of Galileo; there were six contributors and their work groups itself into three areas. The first of these is the importance and relevance of Galileo in modern thought and society: these were discussed by Giorgio di Santillana and Gilgerto Bernardino respectively. Norwood Hanson and E. W. Strong study the work of Galileo in dynamics and his theory of measurement. (...) The last two papers are concerned with the role of science in society—they spring from the confrontation of Galileo and the Church, the first indication of the special problems the scientific disciplines would face in their intercourse with the rest of society—and are by Philip Abelson and Erich Kahler. Although this brief collection cannot provide a full view of the scope of Galileo's activities and their subsequent impact, we are still able to feel that some of the vast gap between ourselves and the initiator of modern physics has been spanned, and that he can be seen more clearly for it.—P. Introduction No consent for health and medical research is appropriate when the criteria for a waiver of consent are met, yet some ethics committees and data custodians still require informed consent. Methods A single-blind parallel-group randomised controlled trial: 1129 families of children born at a South Australian hospital were sent information explaining data linkage of childhood immunisation and hospital records for vaccine safety surveillance with 4 weeks to opt in or opt out by reply form, telephone or email. A subsequent (...) telephone interview gauged the intent of 1026 parents (91%) in relation to their actions and the sociodemographic differences between participants and non-participants in each arm. Results The participation rate was 21% (n=120/564) in the opt-in arm and 96% (n=540/565) in the opt-out arm (χ2 (1 df) = 567.7, p<0.001). Participants in the opt-in arm were more likely than non-participants to be older, married/de facto, university educated and of higher socioeconomic status. Participants in the opt-out arm were similar to non-participants, except men were more likely to opt out. Substantial proportions did not receive, understand or properly consider study invitations, and opting in or opting out behaviour was often at odds with parents' stated underlying intentions. Conclusions The opt-in approach resulted in low participation and a biased sample that would render any subsequent data linkage unfeasible, while the opt-out approach achieved high participation and a representative sample. The waiver of consent afforded under current privacy regulations for data linkage studies meeting all appropriate criteria should be granted by ethics committees, and supported by data custodians. It remains unclear whether such genetic variants are associated with disease aggressiveness. The NCI-SPORE Genetics Working Group retrospectively collected clinicopathologic information and genotype data for 36 SNPs which at the time had been validated to be associated with PC risk from 25,674 cases with PC. Cases were grouped according to race, Gleason score and aggressiveness. Statistical analyses were used to compare the frequency (...) of the SNPs between different disease cohorts. After adjusting for multiple testing, only PC-risk SNP rs2735839 was significantly and inversely associated with aggressive and high-grade disease in European men. Similar associations with aggressive and high-grade disease were documented in African-American subjects. The G allele of rs2735839 was associated with disease aggressiveness even at low PSA levels in both European and African-American men. Our results provide further support that a PC-risk SNP rs2735839 near the KLK3 gene on chromosome 19q13 may be associated with aggressive and high-grade PC. Future prospectively designed, case-case GWAS are needed to identify additional SNPs associated with PC aggressiveness. (shrink)

BackgroundThe ARRIVE (Animal Research: Reporting of In Vivo Experiments) guidelines are widely endorsed but compliance is limited. We sought to determine whether journal-requested completion of an ARRIVE checklist improves full compliance with the guidelines.MethodsIn a randomised controlled trial, manuscripts reporting in vivo animal research submitted to PLOS ONE (March–June 2015) were randomly allocated to either requested completion of an ARRIVE checklist or current standard practice. Authors, academic editors, and peer reviewers were blinded to group allocation. Trained reviewers performed outcome adjudication (...) in duplicate by assessing manuscripts against an operationalised version of the ARRIVE guidelines that consists 108 items. Our primary outcome was the between-group differences in the proportion of manuscripts meeting all ARRIVE guideline checklist subitems.ResultsWe randomised 1689 manuscripts (control: n = 844, intervention: n = 845), of which 1269 were sent for peer review and 762 (control: n = 340; intervention: n = 332) accepted for publication. No manuscript in either group achieved full compliance with the ARRIVE checklist. Details of animal husbandry (ARRIVE subitem 9b) was the only subitem to show improvements in reporting, with the proportion of compliant manuscripts rising from 52.1 to 74.1% (X2 = 34.0, df = 1, p = 2.1 × 10−7) in the control and intervention groups, respectively.ConclusionsThese results suggest that altering the editorial process to include requests for a completed ARRIVE checklist is not enough to improve compliance with the ARRIVE guidelines. Other approaches, such as more stringent editorial policies or a targeted approach on key quality items, may promote improvements in reporting. (shrink)

We take ourselves to have an inner life of thought, and we take ourselves to be capable of linguistically expressing our thoughts to others. But what is the nature of this “inner life” of thought? Is conscious thought necessarily carried out in language? This paper takes up these questions by examining Merleau-Ponty’s theory of expression. For Merleau-Ponty, language expresses thought. Thus it would seem that thought must be independent of, and in some sense prior to, the speech that expresses it. (...) He also claims, however, that thinking just is linguistic expression, and thus that language constitutes thought. The primary aim of this paper is to make sense of this constitutive claim while maintaining that, for Merleau-Ponty, there is an inner life of thought that exists independently of linguistic expression, and that this inner life rightly deserves the label “thought”. The upshot of this account is twofold. First, it explains why the mainstream view of Merleau-Ponty’s theory of expression seems plausible, but is ultimately inadequate. Second, it functions as a corrective to contemporary debates about the nature and scope of phenomenal consciousness and the sense in which conscious experience has content. (shrink)

Historical works on Hitler and Stalin or on specific aspects of their regimes reveal how historians differ in their treatment of individual agency. Historians' practices are examined in the light of W. H. Dray's findings about historians' concepts of causation and A. Giddens's structuration theory. Marxist and revisionist historians rejected approaches that endowed Hitler and Stalin with immense power and personal control over events. Works by Isaac Deutscher, A. J. P. Taylor, and J. Arch Getty exhibit historians' methods for reducing (...) or nullifying agential power. Robert C. Tucker's work on Stalin offers a different approach to the problem of the interaction of structure and individual agency. Allan Bullock may be correct in his view that historians are now less likely to exaggerate or underestimate either individual agency or structure when dealing with Hitler and Stalin; and Christopher Lloyd may be correct to say that historians' practices suggest a tacit acceptance of structuration theory in some form, but it does not follow that historians are now more likely to agree about the agential power of individuals. The assessment of agential power still requires interpretation, and it is doubtful that consensus about structuration theory would affect the range of interpretation very much. However, theories of cultural evolution and comparative investigation of the "selection" of political-cultural "genes" at certain historical junctures might provide a useful framework for studying how individuals like Hitler and Stalin acquire an unusual degree of power and authority. Evans' well-documented article, ‘Wheat production and its social consequences in the Roman world’, correctly makes the point that ‘the evidence with regard to wheat yields is at once meagre and plainly contradictory’. The difficulty in assessing yields arises, of course, from the character of the available source material; namely, literary sources. The information comes from the hands of men such as Cicero and Varro who were concerned with matters other than specific data on the cultivation and production of (...) grains, and who probably never sowed or reaped a modius of wheat. What was lacking until recently was a bona-fide document from the hands of a farmer or a community intimately concerned with the growing of wheat. We now have one such document, P. Colt 82 of the seventh century A.D., that fills a gap in the evidence for yields for both wheat and barley. (shrink)
